Adherence Support Professional

Adherence Support Professional (ASP)

Role Overview: The Adherence Support Professional (ASP) is responsible for ensuring patients follow their prescribed care plans after discharge from a clinical facility or post-outpatient visit. The ASP offers direct assistance, clarifies care instructions, and provides continuous support to patients and their families, ensuring effective engagement with treatment plans to promote positive health outcomes.

Key Responsibilities:

  • Monitor Compliance: Track and document patients' adherence to medications, therapy, and other elements of their care plan, and report any issues or concerns to the healthcare team.
  • Assist with Home Exercise Plans: Guide patients through prescribed exercises, ensuring they are performed consistently and correctly.
  • Provide Reminders: Send reminders to patients regarding appointments, medications, and treatments to maintain adherence to their care plan.
  • Promote Accountability: Support patients in taking responsibility for their health by reinforcing the importance of following their care plan.
  • Coordinate with Healthcare Providers: Communicate patient progress, challenges, and successes to the healthcare team, ensuring that care plans are adjusted as necessary.
  • Address Barriers to Adherence: Identify and help resolve obstacles to adherence (e.g., financial, logistical, or cognitive barriers) by collaborating with patients and families.
  • Provide Emotional Support: Offer empathy and encouragement to patients and families, alleviating any anxiety or confusion related to their care plans.

  • Strong communication and interpersonal skills.
  • Empathy and ability to build trust with patients and families.
  • Understanding of medical terminology, care plans, and basic health conditions.
  • Problem-solving and critical thinking skills, particularly for addressing adherence challenges.
  • Ability to work independently and as part of a collaborative healthcare team.
  • Organizational skills to track and report on patient progress.

Reporting Structure: The Adherence Support Professional (ASP) reports to the Care Plan Adherence Manager and collaborates with healthcare providers (doctors, nurses, therapists) and other care team members to ensure the patient’s health and adherence needs are met.
